Eligibility for services:
1. Consent to admission.
2. Be at least 18 years of age.
3. Ambulatory or mobile non-ambulatory.
4. Be medication compliant.
5. Able to meet own personal needs.
6. Not be a danger to self or others.
7. Follow physician orders.
8. Financial resources for rent/living.
9. Not be in greater need of services than provided by DHI
SCL/Home Based Habilitation.
Consideration for admission is not based on race, creed, color, religion, age, sex, ethnic/national origin, and/or disabling condition.
